چکیده

BACKGROUND Animal models suggest that the neurotransmitter inhibitor, botulinum toxin, when injected into the epicardial fat pads can suppress atrial fibrillation inducibility. The aim of this prospective randomized double-blind study was to compare the efficacy and safety of botulinum toxin injection into epicardial fat pads for preventing atrial tachyarrhythmias. METHODS AND RESULTS Patients with history of paroxysmal atrial fibrillation and indication for coronary artery bypass graft surgery were randomized to botulinum toxin (Xeomin, Merz, Germany; 50 U/1 mL at each fat pad; n=30) or placebo (0.9% normal saline, 1 mL at each fat pad; n=30) injection into epicardial fat pads during surgery. Patients were followed for 1 year to assess maintenance of sinus rhythm using an implantable loop recorder. All patients in both groups had successful epicardial fat pad injections without complications. The incidence of early postoperative atrial fibrillation within 30 days after coronary artery bypass graft was 2 of 30 patients (7%) in the botulinum toxin group and 9 of 30 patients (30%) in the placebo group (P=0.024). Between 30 days and up to the 12-month follow-up examination, 7 of the 30 patients in the placebo group (27%) and none of the 30 patients in the botulinum toxin group (0%) had recurrent atrial fibrillation (P=0.002). There were no complications observed during the 1-year follow-up. CONCLUSIONS Botulinum toxin injection into epicardial fat pads during coronary artery bypass graft provided substantial atrial tachyarrhythmia suppression both early as well as during 1-year follow-up, without any serious adverse events. Botulinum toxin injection in epicardial autonomic ganglia temporarily suppresses vagally mediated atrial fibrillation.

BACKGROUND Autonomic denervation may suppress atrial fibrillation (AF) vulnerability. This study was designed to assess the short- to mid-term effects of botulinum toxin, a cholinergic neurotransmission blocker, on AF inducibility. METHODS AND RESULTS A total of 23 mongrel dogs were studied. The sinus node and atrioventricular node epicardial fat pads were exposed through a right lateral thor...

Short-Term Autonomic Denervation of the Atria Using Botulinum Toxin

BACKGROUND AND OBJECTIVES Major epicardial fat pads contain cardiac ganglionated plexi (GP) of the autonomic nervous system. Autonomic denervation may improve the success rate of atrial fibrillation (AF) ablation. This study was designed to elucidate the acute effects of blocking the right atrium-pulmonary vein (RA-PV) and left atrium-inferior vena cava (LA-IVC) fat pads on the electrophysiolog...

Abstract Background and purpose: To determine the effect of intrasphincteric botulinum toxin injection to treat internal anal sphincter achalasia. Materials and methods: Twenty patients (11 boys, average age 4.95± 2.47 years) with definite diagnosis of internal anal sphincter achalasia received 20 units of botulinum toxin injection in 4 points of the internal anal sphincter (totally 80 unit...

The effects of Vitamin C on Inflammatory Markers and Atrial Fibrillation in Patients Undergoing Coronary Artery Bypass Surgery in Bushehr Heart Center: A Randomized Controlled Clinical Trial

Background: Coronary artery bypass graft surgery (CABG) with cardiopulmonary bypass pump may cause systemic inflammatory reactions by releasing cytokines which leads to multiple organ failure. Vitamin C has anti-inflammatory and antioxidant properties. The study aimed to determine the effect of vitamin C on inflammatory markers and atrial fibrillation in patients undergoing CABG. Materials and ...

INJECTION OF BOTULINUM TOXIN IN THE TREATMENT OF ACHALASIA: A RANDOMIZED, DOUBLE BLIND, CONTROLLED CLINICAL TRIAL

Our aim was to evaluate the short and long term efficacy of botulinum toxin therapy in Iranian patients with achalasia. In a randomized, double blind trial, 20 patients with achalasia, referring to Imam Khomeini Hospital, received either 80 units of botulinum toxin (BT) or placebo (PL) from 1995 to 1998. Two weeks later, the response to treatment was assessed on the basis of changes in the...
